Release of Liability, Assumption of Risk & Indemnification

In consideration of being permitted to participate in activities at KC Sports Facility ("the Facility"), including but not limited to soccer, training, pick-up, leagues, camps, tournaments, parties, and use of the premises and equipment (collectively, the "Activities"), I, the participant (and the parent/legal guardian if the participant is under 18), acknowledge and agree to the following:

1. Assumption of Risk. I understand that the Activities involve inherent and other risks of physical injury, including sprains, strains, fractures, concussion, heat illness, collisions, and in rare cases serious injury, permanent disability, or death. I knowingly and voluntarily assume all such risks, both known and unknown, and assume full responsibility for participation.

2. Release and Waiver. To the fullest extent permitted by law, I release, waive, and discharge the Facility and its owners, operators, tenants, officers, employees, coaches, volunteers, and agents (the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death sustained in connection with the Activities, whether caused by negligence of the Released Parties or otherwise.

3. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost (including attorney's fees) they may incur arising out of my participation, whether caused by my negligence or otherwise.

4. Fitness & Medical Authorization. I affirm that the participant is physically fit to participate. In the event of injury or illness, I authorize the Facility to secure emergency medical treatment, and I accept financial responsibility for any such treatment.

5. Rules & Conduct. I agree to follow all facility rules and staff instructions, and I understand that participation may be suspended for unsafe or inappropriate conduct.

6. Governing Law & Severability. This agreement is governed by the laws of the State of Kansas. If any provision is held invalid, the remaining provisions remain in full force. This agreement is binding on my heirs, executors, administrators, and assigns.

7. Acknowledgment. I have read this entire Release of Liability. I understand that I am giving up substantial legal rights, including the right to sue, and I sign it freely and voluntarily.
